Two types of statutory declarations are required.
A statutory declaration must be provided by all individuals who conduct lobbying activites. In addition each company CEO must provide a statutory declaration about the company's practices regarding success fees.
Please ensure all sections of the statutory declaration forms are completed and that the declarations are witnessed by an appropriately qualified person (who can witness statutory declarations in Victoria PDF, 15kb). Incomplete statutory declarations may delay the processing of an application for inclusion on the Register of Lobbyists.
Completed Statutory Declaration(s) can be mailed to:
Register of Lobbyists
State Services Authority
3 Treasury Place
Melbourne VIC 3002
Further information about the requirement to provide statutory declarations is provided in clause 9.1 of the Code of Conduct.
